Intracytoplasmic Sperm Injection (ICSI) is a specialized procedure used in fertility treatments, especially when addressing male infertility. Often performed as part of in vitro fertilization (IVF), ICSI is designed to help achieve fertilization when conventional IVF methods may not succeed due to sperm-related issues.
ICSI is primarily used when there are challenges related to sperm quality, quantity, or motility. Specific scenarios where ICSI may be recommended include:
ICSI follows a series of carefully controlled steps and is usually performed under a microscope by a skilled embryologist:
Ovarian Stimulation and Egg Retrieval: Before ICSI, the female partner undergoes ovarian stimulation with hormones to produce multiple mature eggs. These eggs are then retrieved from the ovaries in a minor surgical procedure.
Sperm Collection and Preparation: A semen sample is collected, and healthy, motile sperm are selected. If necessary, sperm can be surgically retrieved directly from the male’s reproductive tract.
Injection of Sperm into Egg: Under a high-powered microscope, an embryologist uses a very fine needle to pick up a single sperm and directly inject it into the cytoplasm of a mature egg. This bypasses natural barriers, such as the egg’s outer layer (zona pellucida), which sperm may struggle to penetrate in cases of male infertility.
Monitoring and Embryo Culture: The fertilized egg (now called a zygote) is placed in a culture environment and monitored to check for successful fertilization and early embryo development.
Embryo Transfer: After a few days of development, one or more healthy embryos are selected for transfer into the woman’s uterus. Any additional embryos can be frozen for future use.
ICSI can dramatically improve the chances of fertilization when male infertility is a concern. By ensuring that each egg receives a sperm, ICSI can often overcome challenges that may prevent fertilization naturally or through traditional IVF.
While ICSI does not guarantee successful pregnancy or birth, it has a high fertilization rate, typically between 50-80%, depending on the specific fertility factors at play. Many couples who may not have been able to conceive naturally or through IVF alone have achieved pregnancy with ICSI.
